To meet the Medicare paperwork requirement for prescribing power mobility equipment, you must submit the following items to The SCOOTER Store prior to delivery:
- Completed Face-to-Face Evaluation Form
- Pertinent Chart Notes (must indicate that the reason for the visit was mobility-related)
- Prescription for Power Mobility Equipment (include necessary medical accessories)
