How to obtain your CEU from the SCOOTER Store:

1. Register

   Complete the registration form below.

2. Read the Article

   Click to select and download your choice of CEU article.

3. Complete the Activity Evaluation

   The test and evaluation over the material are the last items of the PDF document. When you have completed your study of the article, print out both the test and the activity evaluation.

4. Mail the Completed Activity Evaluation to The SCOOTER Store

   Please complete the test and Activity Evaluation, put your name at the top of each page and mail both to The SCOOTER Store at the address below:

5. Receive your Certificate of Completion

   Once we have received and reviewed your test, The SCOOTER Store will mail you a Certificate of Completion. If for some reason you do not receive your certificate, please contact Tresa Wells at The SCOOTER Store at 830-627-4333.

6. Mail your Certificate of Completion to Commission for Case Manager Certification (CCMC) or to your state's department of nursing credit.

   CCMC has accredited this program for one contact hour for Certified Case Managers, (CCM). Each state department of nursing will determine the amount of credit given to RN's and LVN's, (most states mirror the same amount of credit as issued by CCMC since it is a national accrediting agency).
